Case Coordinator
Responsibilities: Monitor and coordinate service delivery to clients and supervise home care workers. Coordinate care in a team approach with nurses and home care workers. Communicate with clients to determine quality of services. Report to certified agencies in accordance with contractual requirements. Maintain accurate and timely documentation. Assist compliance department with in-service information to workers.

Qualifications: High school graduate or equivalent; AA and/or experience in home care or human services preferred. VNS experience desired. Computer experience necessary. Excellent customer service skills; good written and verbal communication and time management skills. Bilingual a plus.

POSITIONS IN COMMUNITY SERVICES  
   

Director of Youth and Family Services
The Youth and Family Services Department has a budget of $2.6 million and 150 staff. The department serves over 3,000 children and young adults ages 5 to 21 each year.

Responsibilities: Tthe director of Youth and Family Services will manage all department programs, including direct supervision of 10 program directors; represent youth programs externally; and expand the department with revenue-generating services. The director will identify and respond to community needs for youth services; communicate a vision for the youth programs that drives planning and program development; assure a culture of excellence and accountability to young people and the community; build a comprehensive, integrated approach to youth development; and facilitate teamwork among sites.. .

The director will be responsible for programs including college readiness, adult ESL, job readiness, elementary after-school programs, a Beacon community center, middle school Options, parent engagement, and family literacy. Some programs operate on Saturdays, during the summer, and during school breaks. He/she will collaborate with agency leadership to develop and implement a fundraising strategy, including proposal development and grant writing; collaborate with managers and fiscal staff to develop and manage program budgets and finances; monitor the status of public funding contracts and assure timely and accurate program and fiscal reporting.

Requirements: Minimum of 2 to 3 years of managerial experience in a similar work setting; knowledge of youth development principles and urban, multicultural environment; genuine desire to shape, develop, and motivate a strong team of diverse program supervisors and staff to pursue a shared programmatic vision; experience with government contracts, foundation grants, budgeting, and fiscal management; MSW or masters degree in related field preferred.

Should have great communication skills, both oral and written; should be a self-starter, able to work independently; enthusiastic and dedicated to improving the lives of community members; organized and effective problem solver

Case Management

The Case Management Department assists physically and or cognitively frail older adults to maintain independent and safe living in the community by linking them to community resources and services. SCS has several case management positions to fill. These include:

 

Case Management Supervisor
Responsibilities: Provides direct, weekly supervision of case management staff. Ensures that case managers advocate appropriately for clients regarding benefits, entitlements, mental health, and medical care. Provides instruction to case managers and file review in accordance with DFTA’s case management standards, guidelines and time frames. Handles Intake duties, screens requests for services, determines client eligibility, and provides information and referral as appropriate. Conducts home assessments as needed. Keeps program resources current and maintains contact with community/government agencies. Performs other duties, as assigned.

Qualifications: MSW or BSW/BA and minimum of two years supervisory experience in human services field; excellent verbal and written communication skills; computer literacy; solid organizational skills; good interpersonal skills with an ability to establish rapport with staff, clients, and other community and government agencies. Must enjoy working with older adults.

 Case Managers
Responsibilities: Conduct home visits to determine needs of the clients and offer assistance with obtaining services needed by the clients.

Qualifications and experience : BA required, BSW preferred, experience providing case management services strongly preferred. Bilingual Spanish is preferred and fluency in another language is desirable. A licensed driver with a vehicle is a must.

POSITIONS IN ADMINISTRATION AND DEVELOPMENT

Budget Analyst
Responsibilities: Maintain, control, analyze, and prepare reports on the organization’s restricted grants and contracts from state, city, and private sources and prepare budgets for all restricted grant and contract proposals. Enter grant and contract budgets using FUND E-Z and verify accuracy. Review grant and contract invoices and payroll allocations to ensure that they are allowable within line budgets. Prepare appropriate reimbursement claims or reports. Generate internal reports of budget to actual for project management. Reconcile claims to books. Prepare grant and contract budget modifications as necessary. Develop relationships with funding agencies and follow up on discrepancies and procedures. Liaise with agency and contract auditors to resolve audit issues.

Qualifications: BS in business or related field and three years’ experience in nonprofit budgeting, budget analysis, and contract claim preparation. Well organized, strong analytical skills, good written and verbal communication skills. ADP payroll and FUND E-Z software experience desired.

Bookkeeper
Reports to: Deputy Assistant Director of Administration

Responsibilities: Perform tasks related to billing, payroll, and general bookkeeping for home care organization with 800+ caseload. Research discrepancies in billed hours and automated payroll system. Update variance and payroll reports. Assist field operations personnel in resolving home care workers’ payroll problems. Research and resolve miscellaneous billing issues. Record transactions and payment of bills.

Qualifications: Prior experience and accounting degree preferred. Excellent communication and analytic skills. Experience with nonprofit and HRA contracting a plus.
