SERVICES FOR THE HOMEBOUND

Together, Sunnyside Community Services and our two licensed home care agencies, Sunnyside Citywide Home Care Services, Inc. (SCHCS) and Sunnyside Home Care Project, Inc. (SHCP) provide a continuum of care for homebound individuals and their families. Most critical is the skilled and caring home care service, which enables our clients to remain in their homes as independently as possible for as long as possible.

Workers are trained to provide support at whatever level a client needs, but to help and encourage the client to do as much as possible for herself or himself. In addition, we provide case management services to help clients find and gain access to appropriate home care services, and, in Western Queens, provide Meals on Wheels, transportation, and assistance for caregivers of frail and memory-impaired seniors.

 

SUNNYSIDE CITYWIDE HOME CARE SERVICES and
SUNNYSIDE HOME CARE PROJECT

Sunnyside Citywide Home Care Services, Inc. (SCHCS) and Sunnyside Home Care Project, Inc. (SHCP) are committed to the highest-quality in-home care. Each week, our home health aides, home attendants, nurses, and other professionals provide more than 52,000 hours of care each week (help with shopping, cooking, dressing, bathing and personal care, housekeeping, etc.). A plan of care is developed for each client, in consultation with the client, the family, and the physicians; a nurse oversees the implementation of each plan of care.

Home care workers who speak the client's language often can be assigned if the client wishes.

Home care workers receive intensive orientation and training at the beginning of their employment, and ongoing in-service training throughout each year.

CASE MANAGEMENT SERVICES

Case management services include information about and referral for home health care, long-term care, Meals on Wheels, assistance with Medicaid, food stamps, and other benefits.

OTHER SERVICES
FOR HOMEBOUND INDIVIDUALS

Homebound individuals also can receive transportation through our Senior Center. Caregivers of frail and memory-impaired elderly can receive information, counseling, and other assistance through the Western Queens Caregivers Network.
